Pennie is the state-based health insurance marketplace that’s exclusively for Pennsylvania residents.

Middle Child and Domenick Financial recommend part-time employees use this marketplace.

If for some reason you do not qualify for a subsidy, Middle Child will subsidize a portion of the premiums for your health insurance. Can I ask a stupid question?

There are no stupid questions; every query you have is important to us. We’re here to help you with any concerns or information you need, no matter how simple or complex it might seem.

Where do I go to find out which dentist, doctor, emergency room, etc. are in the network? Where is this information listed?

Sign up or log in through the company-specific mobile app or website to access the link to find provider information.

When do I use Vitable and when do I use my health insurance provided by MC?

All preventative care, as per Pennsylvania state law, is covered by your health insurance. Facility fees may apply. Anytime you have to see a doctor for a non-preventative medical issue, it will generally include out-of-pocket expenses. For a list of covered preventative services through your insurance go to: https://www.healthcare.gov/coverage/preventive-care-benefits/

For non-preventative care that does not require treatments, Vitable includes as part of your subscription fee.

How do I file a claim for the accident coverage?

The accident claim form can be accessed on the Middle Child benefits page on their website here. Once filled out, submit it to Domenick Financial, and they will connect you with Colonial to start your claim process.

Can we add dependents on the benefits?

Per the Middle Child benefits plan, dependents can only be added to managers’ plans. If you need to add a dependent, please schedule a meeting with Domenick Financial.

Where can I access mental health benefits?

Depending on what health insurance plan you have, you may have a copay to see a therapist. However, to access short-term therapy solutions, Vitable offers a 10-week program with bi-weekly sessions.

How do I know if my prescription medication is covered?

Your prescriptions could be covered by either your health insurance and/or Vitable. To check to see if your prescriptions are covered and the cost of the coverage, refer to the health insurance app or reach out to Domenick Financial to check your coverage with Vitable.

How are emergency services covered under our plan?

They are covered under health insurance, but depending on what plan you have and which hospital you go to, you may have out-of-pocket costs or copays associated with your visit. If you opted into the accident plan, and if the emergency is due to an accident, you can file a claim on the accident plan to be reimbursed for out-of-pocket costs for utilizing emergency services.

Is telehealth included in my health insurance plan? What are the benefits of a Telehealth visit?

Yes! Telehealth can be accessed through your health insurance or Vitable. The purpose of telehealth is to meet with a doctor for more immediate issues that do not require an in-person visit. This is generally a cheaper alternative to in-office appointments and can usually be scheduled faster.

Can I change my coverage plan mid-year?

You cannot change health insurance mid-year unless you have a qualifying life event. If you have questions about specific qualifying life events, contact Domenick Financial.

  1. Notify Us:Notify your manager of the incident right away so that Middle Child can make an “incident report” for future reference, and assist in getting you the help you need! Make sure you note what happened, when it happened, the cause of the injury, and the exact body part injured.
  1. Head to a Healthcare Facility:When at the healthcare facility, you should provide them with the Worker’s Compensation insurance information provided to you during onboarding, included below for your convenience. You likely will not have a “claim number” yet, but you can reference the policy number instead (44 WEC BV9TJT). You do not need a “claim number” to receive treatment.

  1. Bills / Reports:Take any bills and reports home with you, and send a copy of them to your Team Manager, General Manager and Ownership. This includes any information about how long you’ll have to be off from work or what limitations you’ll have when do you return. If you receive any more bills from the healthcare facility, continue sending them to your Team Manager, General Manager and Ownership. It’s best to follow up to your original email so they’re all in one place!
  2. Middle Child Reports the Claim:Middle Child will report the claim to the insurance company  HERE. The more information we have from you, the faster and smoother this process goes.
  1. The Employee Registers Their Information:When you are feeling up to it, go online and register for “My Workers’ Comp Connection | The Hartford”. This is an online tool to help our insurance providers get in touch with you, if needed.
  1. Wait To Hear Back and Gameplan:The insurance company will begin looking into the claim and determining reimbursement. Be patient, it takes time. And don’t pay any bills in the meantime! If you have to miss work as a result of your injury, you’ll work with the Team Manager, General Manager and Ownership to come up with a plan for coming back.
